Insurance Information
The
Laser Center of Maryland offers its patients many types of treatment. Some
procedures are viewed by insurance companies as cosmetic, while others are deemed
medically necessary. Although we do not participate in any managed
care networks (i.e., HMO, PPO, POS), as a courtesy to our patients, we attempt
to obtain benefits and pre-authorizations for medical procedures before they
are performed. Our office staff completes all medical procedure claims. Payment
for cosmetic procedures is required at the time of treatment.
The Laser Center of Maryland always handles certain insurances in a specific manner.
Below are the brief guidelines for these plans:
BLUECROSS/BLUESHIELD
Payment is required at the time of treatment. Completed claims are sent to the patient for direct submission to BC/BS. Reimbursement is received by the patient directly from BC/BS.
HMOs
If you are required to seek treatment within a network of doctors, you are responsible for payment at the time of service. You will be informed of all charges prior to treatment.
MDIPA, MAMSI, MAPSI, OPTIMUM CHOICE OR ALLIANCE
These companies do provide out-of-network benefits; however, payment is required at the time of treatment. Our office completes claim work and the insurance company makes reimbursement directly to the patient.
PPOs or Open Access
If you have a choice between a network of providers or use of non participating health care providers, an out-of-network deductible usually applies and is collected, as well as any copay, at the time of service. Claims are directly sent to the insurance company for payment.
MEDICARE
Dr. VanAntwerp is a Medicare provider. If you have a supplemental policy, the deductible and remaining 20% will generally be paid. If there is no supplemental policy, the balance is your responsibility. Cosmetic procedures are always collected at the time of procedure.
CHAMPUS
Since we are a non-participator, we ask for payment in full at the time of service. If you are a Champus beneficiary, you agree to waive your rights for balance billing limitations and understand that you are responsible for 100% of the bill.
Financing
Insurance is a complex issue today. At the Laser Center of Maryland we attempt
to provide the best assistance for your reimbursement. We have interest free
financing available and accept Visa, MasterCard, and Discover for your convenience.
